Your Care Plan
You work with your care team create a personalized health care plan. Your family members can also be part of this process. Your care plan will be designed to meet your unique health needs, honor your culture and values, and support your interests and socialization goals. Every Florida PACE Centers physician is experienced in caring for older adults and has a schedule that allows for ample time with every participant.

As Your Needs Change
PACE senior services can change along with your needs. Whether that means fewer or more frequent visits, different medications, additional social support and more. We are here to serve you today, as well as help you in the future.
In case of Emergencies
We have a 24/7 emergency line that you can call if you have a health problem at night or on the weekend. Our on-call nurse will answer the call and, depending on your condition, will arrange for you to see your Florida PACE Centers care team the next day, come out to see you immediately, or recommend a visit to the Emergency Room.
