Enrollment Process
1. Pre-Screening
Get started by finding out if you are eligible. Call one of our Florida PACE Centers Enrollment Specialists at 786.933.7223 or TTY # 800.955.8771. We will walk you through the process and tell you what to expect. If you are not already enrolled in Medicaid, we can help. Typically, the entire process takes two to three months due to the Department of Children and Families’ (DCF) and the Florida Department of Elder Affairs’ requirements.
2. In-Home Visits
There will be three in-home visits. A member of our enrollment team will come to your home to collect the enrollment paperwork and answer any questions you or your family may have about the program. A Florida PACE Centers home care coordinator will conduct the second visit to do an initial evaluation to assess your safety at home. Finally, the State will contact you to schedule an in-home level of care assessment (CARES).
3. Application In Process
While your application is being evaluated, we encourage you to come in for a tour and meet your new doctors and the team providing your care – we will even pick you up, if needed. We know how important it is to feel comfortable with your healthcare providers, and our team is available to answer any questions you may have throughout this process.
4. Customized Care Plan
Once approved for enrollment, we will work with you and your family to personalize your care plan. Our experienced team of providers will evaluate your health, social, and quality of life goals. Our services will adapt to your needs, and you can depend on us for long-term seamless comprehensive care.
